The use of this form is dependant on the number of 'Non-Packed' Medications or Treatments being used by each individual Resident. The Multidose Medication Chart (Ref No. LTHC-02) provides for the recording of one 'Non-Packed' Medication on the inside front cover and these forms provide for additional recording when more than one 'Non-Packed' Medication is being administered to resident. One form is required for each 'Non-Packed' Medication.

The doctor's order for the medication is written into the chart by the Doctor in it's appropriate category: 1. Short Term Medication 2. Regular Medication 3. PRN (When-Required) Medication.

The Medication Order should also be written by the Doctor onto the individual 'Non-Packed' Medication form, or alternatively, a medication label from the pharmacy can be applied. When administering 'Non-Packed' Medication, select the current month, sign the form in the square under the date on the appropriate time line. Each form provides for 6 months administration recording.
